When to use this form:
This form is used by the school district after
consultation with private schools located within the
boundaries of the public school district to determine
appropriate services for children with disabilities
parentally placed in a private school. Under the
provisions of IDEA, a portion of the public school
district’s Part B funds must be expended for services
for students with disabilities parentally-placed in a
private school. This form describes what services will
be provided and the pro rata share of federal IDEA funds
that may be used to provide the services agreed to in
the plan. |
