34. Response to an Activity Requested by Parents/Guardian
  When to use this form:  This form is used by school district administrators/designee to respond to a parent/guardian request to either initiate or not initiate an action regarding their child’s individualized educational program. The form identifies what action the school district proposes or declines regarding the parental/guardian request.

35. Parent/Guardian Consent to Provide Personalized Transportation
  When to use this form:  This form is used by special education staff to obtain parental/guardian consent for the district/cooperative to transport students in vehicles other than the district’s school buses to individualized programs and/or activities. For example: The district uses a private vehicle to transport students to a day school program or preschool children on a field trip.

36. Authorization by Parents/Guardian to Release Personally Identifiable Information to Third Party or Class of Parties
  When to use this form:  This form is used by school administrators/designee to obtain parental/guardian consent to disclose personally identifiable student information to a third party or class of parties. For example: The parents request a copy of the school district/cooperative’s psychological report released to a medical doctor.

37. Notice of Destruction of Information
  When to use this form:  This form is used by administrators/designee whenever the district or cooperative determines that any of its personally identifiable student data is no longer needed. Notice must be given to the student’s parents/guardian of the district/cooperative’s intent to destroy the data. The form gives notice to the parents/guardian, identifies the data to be destroyed, the date of destruction, and appropriate time for the parents/guardian to obtain the data before it is destroyed.

42. Parental Rights and Procedural Safeguards (English Version)
  When to use this form:  This form is a Department of Education document that provides parents/guardian with a description of their procedural safeguards and rights under the Individuals with Disabilities Education Act. Special education staff and administrators are required to give parents/guardian a copy of their rights once a year, upon request, or prior to a due process hearing.
