Destruction of Information |

When to use this form:
This form is used by administrators/designee whenever
the district or cooperative determines that any of its
personally identifiable student data is no longer
needed. Notice must be given to the student’s
parents/guardian of the district/cooperative’s intent to
destroy the data. The form gives notice to the
parents/guardian, identifies the data to be destroyed,
the date of destruction, and appropriate time for the
parents/guardian to obtain the data before it is
destroyed. |
