When to use this form:
This form was developed by the Cooperative to assist
administrators and special education staff during the
referral, evaluation, and eligibility determination
processes. The “Special Education Guide” outlines
each step of these processes, identifies responsibilities,
and refers special education staff and administrators to
selected forms used during the referral, evaluation, and
eligibility stages. The "Special Education Guide" is
an excellent resource for all special educators and
