When to use this form:
The student’s high school special education teacher
completes this form. The form is used to provide a
summary of performance for students on an IEP upon graduation
from high school. The form is given to the student and
the student’s parents/guardian upon graduation from high
school. The form identifies the student’s postsecondary
goals, summary of academic achievement, functional
performance, recommendations to assist in postsecondary
goals, and the student’s input regarding postsecondary
supports and accommodations. |
