When to use this form:
This form is used by the districts general education
teacher(s) that have educational and/or behavioral
concern(s) regarding student performance. The general
education teacher completes the form and submits it to the
districts Assistance Team and administrator prior to
considering a referral for special education services. The
form identifies interventions (backed up by data records)
that the general education teacher has used to help improve
the performance of the student. At least four teacher
interventions are required before a student may be referred.
The administrator/designee conducts the Assistance Team
meeting with the general education teacher(s) making the
referral, the childs parents/guardian, and other qualified
individuals. The Assistance Team reviews the data and
concerns, and makes its recommendations. |
